Enclomiphene is the trans-isomer of clomiphene and acts as a selective estrogen receptor modulator (SERM). It is specifically developed and studied for use in men with low testosterone (secondary hypogonadism) who wish to maintain fertility.

Primary Uses:

Treatment of Secondary Hypogonadism

Enclomiphene increases endogenous (natural) testosterone production by blocking estrogen receptors in the hypothalamus, which stimulates the release of LH and FSH from the pituitary gland.

This leads to increased testosterone production by the testes.

Male Infertility / Fertility Preservation

Unlike exogenous testosterone, enclomiphene does not suppress sperm production.

It is used in men who want to improve testosterone levels while maintaining or improving sperm count and fertility.

Alternative to Testosterone Replacement Therapy (TRT)

Enclomiphene is an option for men with symptoms of low testosterone who want to avoid the risks of TRT, such as testicular atrophy and infertility.

It is especially useful for men of reproductive age.

Enclomiphene is generally better tolerated than clomiphene and has fewer estrogenic side effects.

It is not FDA-approved as of 2025, but is available through compounding pharmacies and clinical trials.

Regular monitoring of testosterone, LH, FSH, and semen analysis is recommended during therapy.
