An initial visit with a provider involves a medical history review, an assessment of your specific symptoms and concerns, and possible diagnostic tests to evaluate a patient's health, identify issues, and develop and discuss treatment options.

For all Mental Health concerns except for ADHD- unless you have past diagnosis and records available.

If you do not have past ADHD diagnosis or if your records are not attached at intake your visit will be canceled and you must register for the ADHD visit.
